About the position

Responsibilities

  • Provide acute, chronic, emergent, and preventive health care services based on training level.
  • Obtain patient history and perform physical exams, implementing care plans as per the Community Health Aide Manual.
  • Complete required documentation with a minimum of 80% proficiency and maintain organized health records.
  • Report patient encounters to referral providers and arrange patient referrals as directed.
  • Ensure confidentiality of health protected information according to HIPAA guidelines.
  • Perform lab testing and documentation of results per TCC procedures and CLIA standards.
  • Provide emergency medical response when on-call and coordinate care for patients.
  • Facilitate patient travel and outreach initiatives for health maintenance.
  • Support technology utilization in service delivery and conduct telehealth equipment checks.
  • Collaborate with various health care professionals and perform clinic management duties.
  • Attend required training and continuing medical education, maintaining EMS certification.
  • Provide health education and disease prevention information to the community.
  • Report monthly to the Village Council on health services and activities.
  • Perform a minimum of 8 patient encounters per month, with potential travel to busier clinics if necessary.
  • Provide itinerant coverage in other clinics as requested by the program.
  • Support compliance with accreditation and regulatory standards, including dress code.
  • Assist with new hire CHA orientation.

Requirements

  • High School Diploma or GED.
  • Must be at least 18 years of age.
  • Pass background check as per Federal Indian Child Protection and Family Violence Prevention Act requirements.
  • Ability to pass TABE at an 8th grade level.
  • Strong verbal, written, interpersonal, and analytical skills.
  • Ability to read, write, comprehend English, and perform math computations.
  • Maintain strict patient and program confidentiality.
