About the position
As a Community Liaison for Sarasota County, FL, you will play a vital role in connecting the community with the services offered by Discovery at Home, a new Home Health agency dedicated to improving the quality of life for patients and their families. You will be responsible for sales, marketing, and educating the community about the agency's services while actively obtaining patient referrals. In this position, you will maintain an in-depth knowledge of the community, healthcare facilities, and referral sources that are appropriate for the agency's services. You will assist the Vice President of Operations in establishing targets and goals for the development of all service lines in the market. Your responsibilities will include developing a marketing plan that identifies priorities and sets a reasonable timetable for achieving these goals. You will be tasked with establishing, growing, and maintaining relationships with facility-based referral sources, ensuring compliance with company policies and procedures. This will involve regular communication with existing referral sources and identifying new opportunities to enhance the agency's outreach. You will visit doctors' offices, hospital discharge desks, assisted living facilities, and other potential sources of referrals to present the agency's credentials and secure patient referrals. Additionally, you will provide in-services and professional presentations for referral sources, community groups, and other organizations regarding the agency's services, programs, and outcomes. Maintaining regular contact with discharge planners, social workers, hospitals, nursing homes, and assisted living facilities will be crucial to facilitating the referral process. You will also assess patients' clinical needs and gather clinical information from hospitals and patients, all while championing the mission and vision of the organization and projecting a positive and professional image in the community.

Responsibilities
- Maintain an in-depth knowledge of the community, health care facilities, and referral sources appropriate for agency services.
- Assist the V.P. of Operations in establishing targets and goals for development of all service lines in the market.
- Develop a marketing plan, identifying priorities and setting a reasonable timetable.
- Establish, grow, and maintain relationships with facility-based referral sources in accordance with Company policies and procedures.
- Visit doctors' offices, hospital discharge desks, assisted living facilities, and other possible sources of referrals to present Agency credentials and obtain patient referrals.
- Provide in-services and professional presentations for referral sources, community groups, and other organizations regarding DAH's services, programs, and outcomes.
- Maintain regular contact with discharge planners, social workers, hospitals, nursing homes, assisted living facilities, and other referral sources to facilitate the referral process.
- Assess the patient's clinical needs and gather clinical information from the hospital and the patient.
- Champion the mission and vision of the organization and project a positive and professional image for the agency in the community.
Requirements
- Ability to set priorities and demonstrate competent organizational skills.
- Healthcare marketing, admissions, or referral experience.
- Ability to handle stressful situations and meet deadlines.
- Excellent oral and written communication, presentation, and interpersonal skills.
- Demonstrate the ability to communicate effectively with all members of the health care team.
- Desire to work in a business development role.
